I usually go by an image I have found and like-especially anything nature related. I had a restoration catalog, found some images of old tubs and made two different pages featuring the tubs. I'll post them here. I think you just to to have a big pile of stuff to work with.

Occaisionally I use the title of the book as the theme. I leave the title on the spine as is and go from there.

I impressed a plate from around an old door knob in terra cotta. Once the terracotta dried and was fired I cast it in paper. It is flat enough to get the idea of a door and its symbology and can be put between the pages of a book (esp if you cut several pages away to allow it to close flat). If applying to the front of a book you you could actually add a vintage door knob.

you could use a drawer pull that looks like a doornob. you can get small hinges at the hardware or craft store.

Cover the page with paint, scraps of paper, or gesso so that it is blank and you can begin fresh. Pick a color, a picture, a word, thought, theme, and then look for other things that are similar or oposite.

Pick your favorite scripture and illustrate what it means to you. Focus on some aspect of the divine and try to describe it via the book. Pick some place you want to go to, or have been and make that your theme.
